New surgical technique to restore a melted cornea in Dohlman-Doane keratoprosthesis type I

Objective
Implantation of a keratoprosthesis often represents the last chance to achieve visual rehabilitation in patients with ocular surface disorders and high risk of graft failure. Nevertheless, postoperative handling of these often by multiple pathologies affected eyes still is a challenge for treating ophthalmologists. Corneal thinning and melting is one of the postoperative complications that require new surgical treatment like implantation of a standard corneal graft or replacement of the device in most cases. Trying to minimize the surgical traumatism we developed a new method of replacing the melted cornea in a carrier of Dohlman-Doane keratoprosthesis type I without extraction of the device.
Methods
Conventional preparation of donor tissue. Three corneal incisions in the central opening at the 3, 9 and 12 o`clock meridian positions allow us positioning of donor tissue between the prosthesis plates and fixation by interrupted 10-0 nylon sutures.
Results
Our patient did not experience any type of complication during a follow-up period of six months.
Conclusions
We consider our technique as a successful treatment of corneal melting without extraction of the keratoprosthesis and avoiding severe operative ocular traumatism, a new surgical approach for this condition.
