Topical Mitomyin C for the treatment of malignant neoplasias of the conjunctiva

Objective
To evaluate the efficacy of amniotic membrane graft transplantation and topical mitomycin-C (MMC) for the treatment of conjunctival squamous cell carcinoma and malignant melanoma.
Methods
In a retrospective study 9 eyes of 9 patients with histological proven sqamous cell carcinoma (n=7) and malignant melanoma (n=2) were evaluated. The tumours were excised and amniotic membrane was used to close the conjunctival defect. Three to six courses of topical MMC 0.02% were applied four times daily for 7 days per course. The patients were followed for 3 to 44 months (median 14 months) after surgery.
Results
Successful conjunctival surface reconstruction and physiologic fornix depth was achieved in 8/9 patients. In one patient shortening of the inferior fornix was observed. Local MMC therapy was well tolerated in 7/9 patients. In one patient partial limbal stem cell deficiency was observed after the fourth course of MMC, in one case local MMC therapy was stopped after the second course because of local intolerance to MMC. Recurrence of primary malignancy was observed in 0/7 patients with sqamous cell carcinoma and 1/2 patients with malignant melanoma during follow-up.
Conclusions
Amniotic membrane transplantation was found to be effective in ocular surface reconstruction after conjunctival tumour excision. Adjuvant topical MMC showed good results in the treatment of conjunctival squamous cell carcinoma. |
