Long-term complications after 106Ru-Brachytherapy for uveal melanoma

Objective
106Ru-Brachytherapy is an established treatment modality for uveal melanoma. The aim of this study was to evaluate in a large cohort with long follow-up the local tumour control rate, secondary enucleation rate and complication rate after 106Ru-Brachytherapy for uveal melanoma.
Methods
We performed a retrospective study in 948 patients (mean age: 61.3 years) with choroidal or ciliary body melanomas primarily treated with 106Ruthenium episcleral plaques from October 1992 through December 2005. The Kaplan-Meier method was used for statistical analysis.
Results
Mean tumor size was 5.1 mm ± 2.1 mm (min: 1.0 mm; max: 12.3 mm) at the time of treatment. 17.7% of eyes showed evidence of ciliary body infiltration and 3.9% had infiltration of the iris. 3.5% of eyes had extraocular growth. The mean follow-up was 4.1 years (0.1-14.1 years). At 5 year after the treatment 55% of patients showed radiation retinopathy, neuropathy was noticed in 44%, rubeosis in 13% and secondary glaucoma in 13%. Local tumour control rates were 91% and 90% at 5 and 10 years, respectively. The eye-preservation was 95.0% and 93.7% at 5 and 10 years, respectively. The reason for secondary enucleation was tumour re-currence in 67% and radiation-related complications in 33%.
Conclusions
106Ru-Brachytherapy in the treatment of uveal melanoma with current inclusion criteria is an effective method with favourable local tumour control and eye preservation rate and an acceptable rate of complications. |
