Relapse of a dermoid cyst of the lacrimal gland

Objective
Dermoid cysts are teratoma accounting to germ cell tumours and develop from dispersed embryonic tissue. They can contain tissue from all three germinal sheets. They are normally benign, but can transform into a malignant tumour. Main localizations are ovaries, tubes, sometimes in the cranial region (mostly intracranial) and the testes. Treatment of choice is surgical resection.
Methods
A 56-year-old male patient presented for further diagnosis of an accidental finding in an MRI. The MRI showed a well located, tumour with sharp edges within the lacrimal fossa (1,8 x 1,3 x 1,8cm). Despite of a local pressure the patient had no complaints. No general diseases were found.
Results
A fast section of the specimen resected diagnosed a developed, regressively transformed, epithelial cyst with giant cell inflammation and some hairs. No malignancy was found. 7 years later the patient presented again because. A MRI showed a 2,4 x 1,4cm mass in the lacrimal fossa between bulb, M. rectus lateralis and lateral orbital wall. The lacrimal gland could not distinguished from the tumour. The tumour was resected interdisciplinary with a lateral orbitomy. The histo-pathological examination diagnosed a relapse of the dermoid cyst of the lacrimal gland.
Conclusions
The case presented demonstrates the rare entitiy of a dermoid cyst of the lacrimal gland. Despite of its benign nature a relapse is possible. Swellings in the orbital region should include dermoid cysts as differential diagnosis. |
