CyberKnife as a new treatment option for large uveal melanomas

Objective
To evaluate the effectivity and safety of the CyberKnife as a new treatment option for large uveal melanomas.
Methods
29 patients (23 m/6 w) with a unilateral uveal melanoma and a apical tumor hight of >6mm were treated with Cyberknife. We evaluated only patients for this study with a observation period of at least 6 months (n=15). The median age was 67 years. The treatment planing included retrobulbar injection, CT scan and NMR. The tumor was than treated with 17 – 22 Gy. Patienten were follwed by ultrsound and CT scan every 3 months.
Results
All 15 patients could be treated under retrobulbar injection. In the standardized ultrasound we found a tumor regression of median 8.0 mm to 7.0 mm. The reflectivity increased from median 50% to 60%. In the observation period we found one patient with a secondary glaucoma. In one patient we found after 2 years a regrowth of the melanoma followed by enucleation.
Conclusions
The stereotactic treatment with the Cyberknife is effective and for the patients a comfortable treatment option for large uveal melanomas which can not be treated by brachytherapy. Longer follow up and larger numbers of patients are necessary to finally evaluate this innovative treatment option. |
