Porocarcinoma of the medial lid angle

Background
Porocarcinoma is a rare malignant neoplasm of the skin and especially of the eyelid region but frequent beside the malignant neoplasms of the eccrine sweat glands.
Patient and method
A nodular lesion with central ulceration of the medial lid angle with an appearance of 3 month was excised in a 64-year-old man in good general condition. Expecting a basal cell carcinoma we performed surgical Mohs procedure.
Results
Histological examination revealed a porocarcinoma with a diameter of 9 mm with tumour cell free margins after second tumor resection. There were no regional or distant metastases.
Conclusions
Porocarcinoma is a malignant neoplasm of the cutaneous sweat gland system. Lesions occur most commonly on the lower extremity. Involvement of the eyelids is rare. Local recurrence and regional or distant metastases have been reported. In case of regional lymphatic spread mortality rate of up to 65% have been reported.
For tumour staging ultrasound of the lymph nodes of the neck, abdominal ultrasound, chest X-ray and quarterly ophthalmological and dermatological examinations are recommended.
