Conjunctival squamous cell carcinoma with orbital involvement

Objective
The group of tumours of the ocular epithelium includes a variety of different benign and malignant tumours. Benign tumours of the conjunctiva are well treatable with monitoring or surgery, whereas malignant tumours must be operated adequately.
Methods
We examined a 85-year old patient with a conjunctival tumour of the left eye.
Results
The patient first presented in november 2008 with a 2-week history of a conjunctival tumour of the left eye. The patient had arterial hypertension and dementia. The examination showed a prominent, purulent conjunctival tumour of the left eye. The visual acuity was 0,4 on the right and 0,7 on the left. The retina was without pathological findings. The patient showed a deficit of abduction and adduction of the left eye. A biopsy of the tumour showed histologically an incipient invasive squamous cell carcinoma with inflammation. Computed tomography showed a tumour lateral of the bulbus with a volume of 20x10x31 mm which could not be exactly separated from the lacrimal gland. The tumour did not infiltrate the bulbus or the orbital bone. The findings were discussed interdisciplinary. Because of the age and the oculus melior situation of the patient we performed a confined bulbus-conserving, operation with postoperative radiotherapy.
Conclusions
Conjunctival squamous cell carcinoma is the most common malignant tumour of the conjunctiva. It grows locally destructive and is often located in the palpebral fissure. Gold standard is the complete excision with tumour free margins. In larger and more diffuse tumours a complete excision may not be possible. Therefore radiation, cryotherapy or local chemotherapy with mitomycin C or Interferon alpha 2b are possible. |
