Correlations between SWAP perimetry and Fourier domain OCT in early glaucoma patients

Objective
Glaucoma is one of the leading causes for irreversible blindness worldwide. The aim of our study is to compare the results and find correlations between the short wavelength automated perimetry (SWAP) and Fourier domain OCT in early glaucoma patients.
Methods
In our study 32 patients with early glaucoma were enrolled. They were examined for visual acuity, tonometry, computer perimetry (Humphrey SWAP 30-2) and ОСТ (RNRL 3.45, NHM4, 3D Disk and GCC).
Results
In 7 patients no changes in the SWAP as well as on the OCT were found. In 4 of them GCC map was abnormal in shape and darker in colour. In 10 patients with IOP between 19-21 mm Hg and no changes in the perimetry, 3D Disk detected local thinning of the neuroretinal rim in the temporal zone. The RNFL 3.45 showed a corresponding thinning of the nerve fibre layer (NFL) in 4 sectors. In 15 patients, with IOP between 21-23 mmHg, local changes in the SWAP were present. ОСТ showed corresponding thinning of NFL in 6 sectors and significant excavation on the surface topography. GCC map was always altered.
Conclusion
Short wavelength automated perimetry (SWAP) and optical coherence tomography are novel methods in correlation with each other. Although SWAP is much more sensitive than the standard perimetry, OCT tests especially GCC map are responsible for finding the first signs of glaucomatous damage, several years before typical visual field defects appear. |
