Corneal endothelial cell loss using Ozil® technology in comparison to traditional phako technology in vitro

Objective
The aim of this study was to investigate the postoperative corneal endothelial cell loss following cataract surgery using either torsional phacoemulsification (Ozil® technology) or traditional phaco technology in an in-vitro model of corneal endothelial cell loss.
Methods
We simulated cataract surgery in an artificial anterior chamber with 15 pairs of human corneas which were unsuitable for transplantation. One cornea of each pair was operated using Ozil® the other one using traditional phako technology (Infiniti®, Alcon Pharma GmbH). Two parameters (energy and duration of surgery) were varied. Following surgery the endothelial cell loss during organ culture was observed for 6 weeks.
Results
For different comparable energy levels the relative postoperative endothelial cell loss was significantly higher following Ozil® compared to traditional phako technology. However, it took almost 2,5 times longer for Ozil® to reach the same energy level as for tradtional phaco, which resulted in 2,5 times the amount of fluid that was washed through the anterior chamber. This might explain the difference in endothelial cell loss. Therefore the endothelial cell loss is currently analysed for comparable surgery times.
Conclusions
For comparable energy levels the duration of surgery was 2,5 times longer using Ozil® compared to conventional phaco probably leading to the a higher postoperative endothelial cell loss in this in vitro model. Whether the endothelial cell damage by the usage of Ozil® technology might be lower for comparable surgery times has to be investigated. |
