Local immunosuppressive medication in a case of Rosacea-Keratitis

Objective
Rosacea is a chronic, ideopathic disease of the face due to sun-exposure. It consists of redness, teleangiectasia, papula and pustula. Sometimes there are also symptoms of the eye and eyelid.
Methods
In May 2006 a 66year old patient presented for the first time. He had been suffering from dry eye for the last 5 years with declining vision. Visual acuity was 0,6 and finger counting. He presented with heavy blepharitis, corneal vascularisation left >> right and nebula on left eye. Because of his dermatologic proven rosacea a diagnosis of rosacea-keratitis on both eyes was made.
Results
Therapy consists of artificial tears, cleaning of eyelids and Doxycyclin 100mg once per day for 3 months. The patient showed no improvement. Therefore we started therapy with Ciclosporin – emulsion 0,5% 3 times per day, resulting in subjective and objective improvement within some weeks. Three months later visual acuity was 1,0p right and 1/30 left. There were also improvements on corneal vacularisation and nebula. Over time the medication of Ciclosporin – emulsion was reduced. At present the dosis is once a day.
Conclusions
Rosacea is a chronic skin-desease which shows improvement after application of local antibiotics and immunosuppressive medication. For the eye medication with Ciclosporin is a therapeutic option but also regular cleaning of eyelids is necessary.
