Spectral-Domain-OCT findings in acute traumatic retinopathy

Objective
To demonstrate high resolution optical coherence tomography in acute traumatic retinopathy.
Methods
A 38-year-old female with unilateral decreased vision in her right eye after blunt ocular trauma was investigated by high resolution optical coherence tomography (S-OCT).
Results
Visual acuity was 20/100 OD and 20/300 OS (due to amblyopia). Fundus examination OD demonstrated a macular edema and preretinal hemorrhages. S-OCT in the area of commotio retinae (reference area: superior inner macula) revealed no thickening of the retina (353 µm, normal: 348 ± 14.9 µm) but low reflectivity of the outer retina and lack of the photoreceptor segment interface (JIO). Retinal pigment epithelium (RPE) was normal and outer limiting membrane was determinable but irregular. Four months later some focal hyperpigmentations could be seen clinically, which corresponded to areas of hypofluorescence in the autofluorescence image. S-OCT in the reference area showed less retinal thickness (235 µm), but persistence of the above described structural changes of the outer retina. Visual acuity had improved to 20/40.
Conclusions
Low reflectivity of the outer retina and lack of JIO represents disruption of the outer photoreceptor segments in commotio retinae investigated by S-OCT. These structural changes are persisting at least after 4 months and are accompanied by thinning of the retina.
