Serous detachment of the macula according to raised intracranial pressure and optic pit

Objective
Case report of a 33-year-old woman with serous detachment due to optic pit and raised intracranial pressure.
Methods
A 33 year old woman presented in our outpatient department after she suffered visual loss in her amblyopic right eye. Visual acuity R: 0,1 L: 1,0. Anterior segment findings were normal as was the left fundus. Right fundus showed optic nerve coloboma with optic pit. OCT scan revealed serous detachment of the neurosensitive retina. Pseudotumor cerebri was suspected as the patient was in her 3rd decade of life, small and rather obese. She was presented to our department of neurology.
Results
Lumbar puncture revealed an intracranial pressure of 28 mm H20. After draining 25 ml of cerebrospinal fluid, visual acuity raised to 0,3. OCT examination showed minor subretinal fluid. No medication was used additionally.
Conclusions
To our knowledge is this the first observation of a serous dteachment secondary to optic pit and pseudotumot cerebri. Raised intracranial pressure is very likely to be the reason for the serous detachment in this case. As far as we know, lumbar puncture is no routine examination in serous detachment secondary to optic pit. It's speculative if this finding is an Explanation why congenital optic pits give rise to subretinal leakage in early adulthood.
