Bevacizumab-assisted vitrectomy for traction PDR-detachment

Objective
The antiangiogenic issues of bevacizumab are well known from AMD treatment. Bevacizumab was used for the therapy of rubeosis iridis successfully. The aim of this investigation was to reduce the intra- and postoperative hemorrhage following the removal of traction membranes using intravitreal bevacizumab (IVB) in eyes with severe traction retinal detachment in proliferative diabetic retinopathy (PDR).
Methods
Ten patients (between 36 y. o. and 53 y. o.) with severe traction retinal detachment, rubeosis iridis and vitreal haemorrhage due to PDR underwent 1.25 mg IVB injection two weeks prior to pars plana vitrectomy.
Results
The iris showed a pronounced regression of rubeotic vessels. The traction membranes could be removed without any complications without secondary haemorrhage during the next 2 months postoperatively in all patients. The retina was reattached completely.
Conclusions
The IVB prior to vitrectomy for severe traction PDR-detachment seems to have a lower rebleeding risk than comparable eyes without IVB and seems to have a positive impact onto the intra- and postoperative progress. We plan a prospective, parallel group, randomized trial. |
