Correlation of ADMA-plasma-concentrations with dynamic retrobulbar blood flow measurements in glaucoma patients

Objective
Asymmetric dimethylarginine (ADMA) has emerged as a novel cardiovascular risc factor and as a possible marker for endothelial dysfunction. Endothelial dysfunction is regarded as a vascular risk factor in glaucoma causing alterations in ocular blood flow. The aim of this study was to evaluate whether ADMA-plasma-concentration in glaucoma patients correlate with alterations in retrobulbar hemodynamics under dynamic challenge to the vascular regulatory capacity induced by changes in body position.
Methods
ADMA-plasma-concentrations in 30 glaucoma patients were determined with enzyme linked immunosorbent assay (ELISA). Color Doppler Imaging (CDI) of retrobulbar blood flow velocities in the short posterior ciliary artery (SPCA), central retinal artery (CRA) and ophthalmic artery (OA) were performed after posture change.
Results
The mean ADMA-plasma-concentration was 0.54 µmol/L (± 0.11 µmol/L) with a range from 0.37 µmol/L to 0.79 µmol/L. Three glaucoma patients presented a high ADMA-plasma-concentration above the reference value of 0.4 – 0.7µmol/L.
The regulatory response in the blood flow velocities following posture change did not show a correlation with ADMA-plasma-concentration (e.g. in the ACPB: PSV R=0,107, P=0,60; EDV R= -0,192, P= 0,35).
Conclusions
Glaucoma is not associated with elevated ADMA-plasma-concentrations in general. In addition no correlation could be detected in the regulatory response in retrobulbar blood flows to posture change with ADMA-plasma-concentration in glaucoma patients. Although elevated ADMA–levels are found in numerous diseases associated with endothelial dysfunction, it does not seem to play a major role in the vascular dysfunction of glaucoma patients.
