Photodynamic therapy combined with intravitreal injection of Bevacizumab as treatment option for patients with chronic central serous chorioretinopathy – a case series

Objective
Current literature depicts different treatment options for chronic central serous chorioretinopathy (medical treatment, focal laser, photodynamic therapy, selective retina therapy, injection of Bevacizumab and others). We report about the use of a combined photodynamic therapy (PDT) and intravitreal injection of Bevacizumab (Avastin®) as a treatment option for 6 patients with chronic central serous chorioretinopathy (CSC).
Methods
6 male patients (average age 46 years) with chronic CSC for at least 4 months and without tendency of recovery were treated as follows: standard PDT laser treatment (λ=689 nm, 83 sec., focus on the central leakage spot) with Verteporfin (5,7 ml Visudyne® over 10 min. i.v.) and intravitreal injection of Avastin® (1,25 mg Bevacizumab) administered within 24 hours. Before, 1, 3, 6, 9 and 12 months after treatment visual acuity, fluorescein angiography (FLA) and OCT examinations were documented.
Results
Mean visual acuity increased from baseline 20/50 to 20/25 after 3 months. All patients showed an improvement in visual acuity of 2-4 EDTRS lines. 2 patients had full recovered vision after 3 months. FLA- and OCT-findings showed a restitution of leakages and subretinal fluid in all cases.
Conclusions
PDT combined with intravitreal injection of Avastin® showed an ongoing improvement of visual acuity in all cases. The treatment was well tolerated and we noticed no complications. Further experience is necessary to evaluate the treatment option of PDT and Avastin® combination therapy in chronic central serous chorioretinopathy.
