Bilateral central retinal detachment after indirect ocular trauma with spontaneous remission

Objective
We analyzed the development and follow-up of macular changes after indirect (contre-coup) trauma by optical coherence tomography
Methods
A 48-year-old woman was examined by optical coherence tomography after suffering a contre-coup trauma.
Results
A 48-year-old woman was involved in a head-on car accident without air bags detonation. The patient complained of decreased vision in both eyes, immediately after the collision. After twenty four hours the patient presented herself in our clinic. The best corrected visual acuity was 0, 8 in the right eye and 0, 6 in the left eye. Slit lamp examination showed no pathological signs in the anterior segment. Dilated fundus examination showed broad rim notches of the left optic nerve head and bilateral macular detachment. The elevation of the inner retinal layer was 561μm in the right eye and 632 μm in the left eye, B-scan ultrasonography showed no posterior vitreous detachment. The patient was treated with nonsteroidal anti-inflammatory ointment, oral nonsteroidal anti-inflammatory drugs and acetazolamide. We periodically examined the patient by optical coherence tomography, 7 days after the accident the retinal pigment epithelium resolved and minimal pigmentary changes remained. Her visual acuity improved to 1, 2 within one year.
Conclusions
Serious retinal changes can occur right after indirect ocular trauma, without involvement of direct force. |
