New surgical procedures for the treatment of glaucomas – A critical overview

Glaucoma patients with medically uncontrollable intraocular pressure or with multiple allergies to locally applied anti-glaucomateous eye drops need surgical intervention to prevent the progression of the damage to the optic nerve head. Traditionally, filtrating operations were first line surgical treatment.
Using the markedly increased conservative possibilities during a long time period, the effect of the following microsurgical technique decreases. Today these techniques often could reach sufficient success rates only by the application of cytotoxical substances which however causes complications.
Besides the lasertherapy the use of implants or space holding substances got a higher importance and non-penetrating procedures like deep sclerectomy became more important recently. Canaloplasty gained in.
An overview about the new procedures is given regarding physiologic-anatomical foundation, surgical treatment as well as the presentation of the own experiences.
The present results confirm the new methods as promising alternatives to conservative treatment.
