Effect of Dimethylenastron in experimental glaucoma filtrating surgery

Objective
To analyze the effect of dimethylenaston a mitotic kinesin Eg5 inhibitor in an experimental setting of glaucoma filtration surgery.
Methods
On 35 chinchilla rabbits (ChBBCH) glaucoma filtration surgery, similar to that performed in clinical practice, was performed. The animals received either no adjuvant, an unilateral subconjunctival injection of Dimethylenastron or the vehicle alone at baseline (1,0µmol, 3.3µmol) and at day 3 and 7 thereafter (3.3µmol). The evaluation of antifibrotic efficacy was performed by clinical response and histologic examination. The animals were killed on day 14, and the eyes processed for histology.
Results
The vehicle was well tolerated. In contrast, Dimethylenastron induced a rigidification of the conjunctiva directly after injection. Except for two cases of transient fibrinous reaction no adverse effects such as inflammation or blurring of the optical media were observed in further course. Bleb scarring occurred within 1 week in all groups. The intraocular pressure correlated with the fibrotic process and reached normal levels within 7 days after surgery in all groups. Histologic examination of the surgical area 14 days after surgery disclosed fibrosis in the control groups which was not significantly reduced in the experimental groups.
Conclusions
The subconjunctival administration of Dimethylenastron in the applied dosages did not reduce the conjunctival scarring and did not improve the surgical outcome of glaucoma filtration treatments in rabbits. |
