Surgical options for eye muscle surgery in third nerve palsy

Uni- or bilateral oculomotor nerve palsies account for about one third of cranial nerve palsies affecting ocular motility.
Due to the large nuclear complex of the third nerve and due to the multiple effector muscles of the third nerve, incomplete palsies are common.
Standard procedures in strabismus surgery such as simple recessions or strengthening procedures often do not yield permanent effect under the conditions of third nerve palsies.
That is why a multitude of special transposition procedures has been developed and proposed by several authors.
We overview the indications for high dosage standard procedures and transposition procedures in the setting of partial or total third nerve palsy.
Special considerations concerning binocular prognosis, possible misinnervations and potential accompanying fourth nerve palsies are discussed.
