Seven years aspheric intraocular lenses – advantage or marketing?

The Tecnis was the first aspheric intraocular lens (IOL) which was introduced 7 years ago. First clinical results were published in 2003.
Today, most IOL producers offer IOLs with an aspheric design which was applicated first in monofocal IOLs and meanwhile also in multifocal and toric IOLs .
The aspheric optic design is different: Some models aim to correct the positive spherical aberration (SA) of the cornea to gain a zero SA of the eye, others have an aspheric design which does not add SA as
conventional IOLs do.
While the arguments for different aspheric optic concepts are discussed controversially, there are still general concerns about the profit of aspheric IOLs. Points of concern are the missing customization of SA, the gain of image quality, the sensitivity against decentration and tilt, and the reduced depth of focus. On the other hand stand results of clinical studies demonstrating improved contrast vision particularly under mesopic conditions, no detectable impairment of defocus curves, and the availability of IOLs with different SA.
With regard to results from the literature and own studies these critical points will be discussed including findings using a new techniques to determine the lens position in the eye.
