Predictability of the postoperative anterior chamber depth of the second eye after IOL-Implantation in dependance from the postoperative anterior chamber depth of the first eye.

Objective
One of the main factors contributing to errors in biometrics is the inaccurate prediction of the postoperative VK-depth after IOL implantation.
Methods
In 212 patients (at almost the same axis length of both eyes), the postoperative anterior chamber depth of the first operated eye was determined. This anterior chamber depth was then put into the biometry calculation of the 2nd Eye. After 6 weeks postoperatively (2nd eye) the deviation from the aimed refraction was determined.
Results
Using the postoperative anterior chamber depth of the first eye for IOL biometry of the 2nd Eye produces a much more accurate biometry with significantly less deviation from the planned refraktion.
Conclusions
If in IOL implantation a maximal accuracy of postoperative emmetropia is necessary, you should begin the non-dominant eye with a aim-refraction in a very low myopic range (avoid any hyperopia). For the upcoming operation of the dominant eye the postoperative anterior chamber depht of the first eye should be used in biometry.
