Anti-VEGF therapy in retinal angiomatous proliferations (RAP) combined with serous pigment epithelium detachment (PED) in AMD

Objective
To evaluate the early functional and morphological responses to intravitreal VEGF inhibitors in patients with RAP associated with pigment epithelial detachment PED. The effects of drugs, ranibizumab, bevacizumab, and pegaptanib, were compared.
Methods
Retrospectively, treatment outcome was evaluated in 70 consecutive patients (54 women, 16 men) newly diagnosed with RAP associated with PED (stage II and III) and treated with intravitreal pegaptanib (17), ranibizumab (38), or bevacizumab (15). The patients were examined at baseline and at 6-weekly intervals after they had received three injections of the drug by refraction protocol best corrected visual acuity (BCVA), fluorescein angiography (FAG), indocyaningreen angiography (ICG-A), and optical coherence tomography (OCT). The results of difference kinds of treatmants were compared after 6 month follow up.
Results
The mean BCVA at baseline was 0.78 logMAR (95% CI 0.71 - 0.86). The mean value of BCVA change after treatment in comparison to the baseline value was -0.07 (95%CI -0.15 - +0.004). The central retinal thickness decreased after treatment to -86.26 µm (95% CI -140.85 - -31.67). The maximum height of PED measured by OCT imaging could be reduced in the mean by -0.97 units (95%CI -1.16 to +0.79, Wilcoxon test p<0.0001). The highest significance was seen after Ranibizumab 2.32 to 1.17 units, p<0.0001).
Conclusions
The early results of treatment of RAP with intravitreal VEGF inhibitors suggest they provide a stabilizing effect on this normally progressive disease. The functional and morphological results after Bevacizumab and Ranibizumab therapy were slightly better than those with Pegaptanib
