Comparison of ranibizumab monotherapy with combination therapy of photodynamic therapy (PDT) plus ranibizumab for the treatmentof neovascular AMD

Objective
We compared the efficay of ranibizumab monotherapy with the combination therapy of photodynamic therapy followed by 3 injections of ranibizumab. Primary end point was change in visual acuity and change in central retinal thickness measured by OCT after 6 month.
Methods
In a retrospective clinical cohort study 91 patients with occult or classic CNV were treated with ranibizumab monotherapy. In a second group 28 patients were treated with combination therapy and received a standard PDT 12 to 24h prior to the first injection. 2 additional injections were administered 4 and 8 weeks after the first treatement. BCVA, OCT and angiography with fluorescein were performend preoperatively and at month 3 and 6.
Results
Preoperative mean BCVA was 0,276 (±0,131) decimal in the monotherapy group and 0,270 (±0,231) in the combination therapy group. After 6 month BCVA was assessed at 0,280 (±0,220) decimal in der monotherapy and at 0,362 (±0,30) in the combination therapy group. The central retinal thickness after 6 month was reduced from 279,8 (±113) μm to 246,7 (±58) μm in the monotherapy group. In the combination therapy group a reduction of CRT from 307 (±91) μm to 285 (±140) μm was found.
Conclusions
The intravitreal injection of ranibizumab and the combination therapy were well tolerated and we observed no severe side effects.
Measurement of central retinal thickness showed a reduction at any assessment point compared to preoperative findings.
The increase in mean BCVA was higher in the combination therapy group than in the montherapy group. |
