Spectral OCT history of macular hole

Objective
To present the formation of macular hole and mechanism of its closure after vitrectomy.
Methods
Both eyes of patients with macular holes were examined with spectral OCT (6 and 3um resolution).
Results
Three different types of macular hole formation were discovered: cystoid spaces in inner retinal layers, vitreomacular traction syndrome and elevation of the photoreceptor layer. Macular hole closure and the recovery of foveal contour can last up to 12 months after surgery.
Conclusions
Spectral OCT due to better resolution and 3 dimensional images allows to visualize that macualr hole often begins with elevation of the photoreceptor layer which is probably due to traction. Postoperative healing begins in inner retinal layers and may be prolonged. Various postoperative defects have been observed.
