Evaluation of vitreo-macular traction syndrome in Spectral-OCT

Objective
Evaluation of microstrutural changes in vitreo-macular traction syndrome (VMTS) in 12 months follow-up study by Spectral-OCT. Evaluation of retinal morphology in the macula was analyzed, with particular attention to corelation between vector forces and vitreous surface adhesion.
Methods
14 eyes of 14 patients with VMTS were investigated with Spectral-OCT in 12 follow-up studies. We observed microstructural changes in the macula and position of the posterior hyaloid membrane.
Results
We classified VMTS into three types: V type (10 eyes), Focal type (3 eyes), H type (1 eye). In the beginning of the study photoreceptors defect was present in 7 eyes, in 3 of these eyes the photoreceptors were elevated. Epiretinal membrane was present in 5 eyes, foveal cavitation in 8 eyes, cystoid macular edema in 7 eyes and in 3 eyes lamellar macular hole was developed.
In 12 months follow-up study in 2 eyes macular hole developed, in one of these eyes the macular hole was spontaneous clossed. In 7 eyes the vitreous was totally detached from the retina and in two of these eyes the lamellar macular hole closed spontaneous. In 2 eyes the vitreous surface adhesion decreased. In 3 eyes macular morphology and vitreous adhesion didn’t change.
Eyes which had vitreous surface adhesion 30-140 µm in the temporal side and 30-140 µm on the nasal side in the first examination, hadcomplete vitreous detachment in the last examination.
In 3 eyes in which macular morphology and vitreous adhesion didn’t change the surface of the vitreous adhesion was 350-950 µm. In 2 of these eyes we decided to performe pars plana vitrectomy with ILM peeling.
The vitreous surface adhesion decreased in 2 eyes. In these eyes the mean surface of vitreous adhesion was 190 µm in the nasal side and 250 µm in the temporal side.
In one eye with V type and one eye with H type the macular hole developed. In two of these eyes the photoreceptors were elevated.
Conclusions
In eyes with surface of the vitreous adhesion less than 140 µm we observed complete vitreous detachment. In eyes with surface of the vitreous adhesion above 350 µm the status of the macular morphology didn’t change. Maybe in these eyes pars plana vitrectomy with ILM peeling should be performed. |
