Intraocular lens and pseudoexfoliation – management of late complications

Objective
Dislocation of an intraocular lens (IOL) with the capsular bag may be a late complication of uncomplicated cataract surgery reported with increasing frequency in recent years.  In 50% of the cases pseudoexfoliation is the predisposing condition. Significant IOL decentration and dislocation may occur spontaneously, after Nd:YAG laser capsulotomy or posttraumatically.
Methods
From the viewpoint of anterior and posterior segment surgery indications for various treatment options are explained.
Results
Pseudophakodonesis should be monitored closely. Early decentration with visual and refractive symptoms may be treated minimally invasive by suture fixation of the IOL. Progredient dislocation needs IOL exchange.
Conclusions
Pros and cons for aphakic correction in case of lacking capsular support (scleral suturing, anterior chamber lens, iris fixated IOL) are discussed.
