Functional results with two multifocal IOLs with different near addition

Objective
The latest generation of multifocal IOLs is gaining increasing acceptance. A point of criticism is the weak performance at intermediate distances (e.g. computer work). Therefore, Alcon developed a ReStor IOL with + 3 D near addition, which was compared to the ReStor + 4 D in a prospective study.
Methods
After uneventful cataract surgery using phacoemulsification, 20 patients received bilateral implantation of the ReStor + 3 D, 20 patients served as control with the ReStor + 4 D in both eyes. After 3 months the following parameters were assessed: Distance visual acuity (VA) uncorrected and bestcorrected, near vision in 40 and 70 cm distance, defocus curves monocular and binocular, and contrast sensitivity.
Results
Uncorrected and bestcorrected VA was very good in the two groops, being significantly superior with binocular testing. Contrast sensitivity also showed no difference between the two IOLs. Significant difference w
as found at intermediate distances: The + 3 D model was slightly better at 40 cm distance (0.7 vs 0.6) and markedly superior at 70 cm at distance (0.8 vs 0.5).
Conclusions
The minimal better reading performance of the ReStor + 4 D at 33 cm is more than counterbalanced by the significantly superior vision with the ReStor + 3 D in the intermediate distance range. This model seems to be more appropriate to be in accordance with the demands of our patients. |
