Intraocular biopsies – Results and efficiacy of Iris and uveal biopsies performed by the Essen biopsy forceps in comparison to other biopsy techniques

Objective
There are several techniques and instruments for performing biopsies of intraocular solid masses. The Essen biopsy forceps was introduced as an alternative approach. The results and efficiency concerning getting diagnosis should be evaluated and compared to literature.
Methods
In a retrospective study all biopsies (Iris/uvea) performed in our oncology service during 2000-2008 were analized. Age, tumor anamnesis, tumor dimension, biopsy instrument (biopsy forceps vs vitrector) and result were evaluated.
Results
A total of 101 uveal biopsies were performed, 57 by biopsy forceps (gr.A) and 44 by vitrector (20G/23G/25G) (gr. B).
Mean age in group A was 72 years. 15 Patients had a systemic tumor anamnesis. Tumor height in mean was 4,2mm. There was informative material in 51 Patients (89,5%) with a.o. 33 Patients uveal melanoma, 6 uveal nevi and 5 uveal metastasis. Mean age in group B was 64 years. 15 Patients had a systemic tumor anamnesis. Informative specimen was found in 33 (75%) patients with a.o. 17 cases of uveal melanoma, 5 uveal metastasis and 4 intraocular lymphoma.
Iris biopsies were performed in 57 cases. Vitrector as biopsy instrument was used in 41 (gr C) and biopsy forceps in 16 cases (gr. D). Mean age in group C was 53,5 years and 59 years in group D. Tumor thickness in group C was 1,53mm with a tumor dimension in mean (hours) 2,94, In group D tumor thickness was in mean 1,82mm with a dimension of 4,5 hours. Informative material was obtained in 31 (75,6%) cases in group C and 13 cases (81%) in group D. Iris melanoma was found in 19 cases in group C vs 9 cases in group D.
Conclusions
Transretinal biopsy of intraocular solid tumors using the vitrector and especially the Essen biopsy forceps is a safe and informative procedure. Further studies are recommended to confirm the results. |
