Serologic diagnostic in patients with retinal vein occlusion

Objective
The Pathogenesis of retina vein occlusions (RVO) is still not completely clarified. The most important risk factor is the endothelial dysfunction resulting from hypertension, diabetes mellitus, smoking, hyperlipidemia and hyperhomocysteinemia. Still discussed is the role of thrombophilia. Because of limited number of investigated patients, studies investigating the role of hypercoagulable states in patients with RVO have brought very incoherent data.
Methods
A literature search using the electronic literature databases (Medline and Ovid) was used to find eligible studies reporting on prevalence of antiphospholipide syndrome (APS), Factor V Leiden-mutation (FV Leiden), hyperprothrombinemia, deficiency of protein C, protein S, and deficiency of antithrombin III. A meta-analysis for each single disorder was performed. The review of current results and recommendation for clinical praxis is presented.
Results
In the meta-analysis, showed statistic significant higher prevalence of APS, hyperhomocysteinemia and the FV Leiden in patients with RVO compared to healthy population. For other analyzed thrombophilic risk factors, no statistic significant differences were found.
Conclusions
The general screening of all thrombophilic disorders in all Patients with RVO is not indicated. Only antiphospholipide syndrome according to valid diagnostic criteria should be excluded in all patients. The investigation of thrombophilia (especially FV Leiden) is recommended only in young patients and in patients with systemic risk factors (hypertension, hyperlipidemia, and diabetes mellitus).
