Trabecular aspiration in exfoliative eyes – a summary

Objective
Trabecular aspiration (TA) has been introduced to the repertory of glaucoma surgery in exfoliative eyes more than 10 years ago.
Methods
A literature search in Pubmed using the terms „trabecular aspiration“ and „glaucoma“ and a retrospective analysis of all eyes undergoing combined phaco+TA in our department of Ophthalmology between 2003-2005 were performed.
Results
Two prospective studies showed a significant reduction of the intraocular pressure (IOP) following single trabecular aspiration in PEX glaucoma. However, this effect was only temporary lasting for several weeks or months. Two other prospective studies proved a significant stronger IOP reduction following combined phaco+TA compared to single phaco in PEX eyes. Our retrospective data confirmed a statistical correlation between the maximum IOP in patient´s history and the failure rate following phaco+TA.
Conclusions
Combined phaco+TA is a valuable tool for surgical IOP reduction in PEX eyes if the patient´s maximum intraocular pressure is only moderately elevated (<28 mmHg). Single TA in PEX eyes – without phaco – is not recommended as a standard procedure.
