EDMAGS II (Evonik-Degussa-Mainz-Glaukom-Studie): Sensitivity and Specificity of IOP and FDT

Objective
In a Glaucoma Screening project employees of Evonik industries were examined. A Glaucoma specialist assessed and categorized the findings. In Comparison to this assessment Sensitivity and specificity of IOP alone, FDT and IOP, FDT or IOP were calculated.
Methods
4234 employees of the company Evonik industries in the age between 40 and 65 years were examined. Intraocular pressure (IOP), visual field (FDT, C20-1) and assessment of the photography of the optic nerve head (Canon CGI) were the base of categorization in non-suspect, glaucoma suspect and highly glaucoma suspect by a complex algorithm.
IOP values of > 21 were classified as conspicuous.
In a statistical model sensitivity and specificity of increased IOP alone (1), pathological FDT or increased IOP (2) and pathological FDT and increased IOP (3) were determined.
Results
16 employees could not be taken into account due to lacking FDTs. Sensitivity and specificity of (1) were accounted for 55.5% and 92.7% in glaucoma suspects and 61.5% and 91.7% in highly glaucoma suspects. (2) had 64.9 and 88.3 in glaucoma suspects and 92.3% and 87.2% in highly glaucoma suspects. In (3) sensitivity and specificity were 6.4% and 99.7% in glaucoma suspects and 15.4 and 99.6 in highly glaucoma suspects.
Conclusions
Neither IOP alone nor IOP in combination with FDT achieved sufficient values in both parameters in comparison to the categorization of the glaucoma specialist. Regardless of this the predictive values are to be considered. They are constantly insufficient and their values are always less than 20%.
