Advanced personalized treatment with active cyclotorsional eyetracking (ACE)

Objective
To evaluate the clinical performance of the wavefront-guided ablation mode (Technolas Zyoptix Advanced Personalized Treatment) in LASIK utilizing active cyclotorsional eyetracking (ACE) and the Advanced Nomogram (adjusting intended myopia correction according to existing higher order aberrations) and wavefront-sensing without pharmacological pupil dilation (NoDiZy).
Methods
Single-center study including 138 eyes. Eligible eyes were between 0 D and -9.25 D of sphere and below 5 D of cylinder. Pupil dilation for wavefront-sensing was achieved with tropicamid 5% eye drops.
Both eyes of a patient were treated on the same day. Mean outcome measures: Influence of medically induced mydriasis on predicted phoropter refraction (PPR) at Zywave-measurement, high contrast visual acuity with and without correction, predictability of the spherical and cylindrical correction, efficacy and safety.
Results
After applying mydriatic eye drops a hyperopic shift of the PPR of up to 1.5 dpt (mean, 0.23 dpt) was seen. Active cyclotorsional eye tracking was dependant on sharp quality of iris image taken at the Zywave-aberrometer. No serious adverse effects were observed. The efficacy index was 1.05 after 15 months of application. The safety index was 1.12. The uncorrected visual acuity leveled off at 0.8 (0.4 to 1.25, decimal notation) at 3-month follow-up. The mean spherical equivalent was calculated to be -0.14 D ± 0.5 D. 78.3% of the eyes were within ± 0.5 D and 95.7% were within ± 1.0 D of the target refraction.
Conclusions
Results indicate a safe and effective overall clinical performance of Zyoptix Advance Personalized Treatments. Active cyclotorsional eye tracking and wavefront-sensing without mydriatics help to avoid refractive surprises |
